Who can access Your Care Connected at the above organisations?

We have worked closely with our colleagues in secondary care, as well as patients and GP practice staff, to come up with the following list of healthcare professionals who will be able to access patient information via Your Care Connected at the participating organisations.

It is important to note that access to Your Care Connected will only be provided where the person has secure access to the hospital's existing clinical system, they are directly involved in the patient's care and the patient is present at an appointment and has given permission/the 'break glass' procedure has been used in the event of an emergency.
