What is Your Care Connected?

Your Care Connected is a new electronic system which will allow doctors, nurses and other authorised health and care staff to view relevant information from a patient’s GP record to provide better, safer care.

The programme is managed by Birmingham and Solihull CCG in a partnership with Sandwell and West Birmingham CCG.

What are the benefits?

The benefits of the system are:

  • Faster and easier access to medical information, which could save lives
  • More secure than the current paper-based access
  • Improves the safety and quality of patient care
  • Gives patients more control over their care
  • Enables safer delivery of emergency care
  • Less time spent on the phone and fax providing or requesting patient information
