What information is shared?


The following information is shared using Your Care Connected

Health professionals treating you at one of the participating NHS organisations will only view the parts of your medical record listed below and only with your permission. If you agree to give them access for the purposes of treating you, it provides them with a host of important information, including:

  • Allergies
  • Medication
  • Hospital admissions and referrals
  • Vaccinations and immunisations
  • Test results
  • Diagnoses                                                                                       
  • Treatments
  • Medical procedures
  • End of life wishes
  • Personal information (age, gender etc.)
  • Concerns related to abuse of vulnerable children or adults (if applicable)

The following information is NOT shared using Your Care Connected

Your Care Connected does not share everything from your GP record. This is because we follow national guidance on patient confidentiality and the sharing of sensitive information. As a result, Your Care Connected does not currently share any information relating to:

  • IVF treatment
  • HIV and AIDS
  • Sexually transmitted infections
  • Gender reassignment
  • Termination of pregnancy

Free text from your GP record - that is, the notes your GP makes in your record when you visit them - is also not currently shared using Your Care Connected.

Please note: these lists may be subject to change to reflect changes in national guidance on the sharing of sensitive information. All changes will be reflected on this website as and when they become available.
